How to qualify for medical assistant work in South Africa
South Africa has two roles covering the ground a US medical assistant would: Clinical Associate, registered with the HPCSA, and Nursing Auxiliary, enrolled with the SANC. They are quite different from each other and from the American job. Certified Medical Assistant is a US credential from the AAMA and there is no South African register that recognises it, so anyone arriving with a CMA qualifies from scratch through one of the two routes below.
Clinical Associate
Registered with the Health Professions Council of South Africa under the Health Professions Act 56 of 1974. The qualification is the Bachelor of Clinical Medical Practice, a three-year degree offered at Wits, the University of Pretoria and Walter Sisulu. Scope of practice is set by Government Notice R.433 of 25 May 2015. The protected title and abbreviation are Clinical Associate (ClinA). This is the closer analogue to a US physician assistant than to a medical assistant.
Nursing Auxiliary
Enrolled with the South African Nursing Council under Regulation R.2176, after completing SANC-approved training at an accredited nursing school and passing the required examination. Shorter, and nursing rather than clinical practice.
What catches people out
The verbs are not interchangeable and getting them wrong marks you out immediately. Registered nurses are registered with the SANC. Nursing auxiliaries are enrolled with it. Clinical associates are registered with the HPCSA — a different council, under a different Act.
Medical assistant salary in South Africa
Where you work decides how you are paid. Public-sector posts follow the DPSA salary scales, published in Circular 15 of 2026; private-sector posts are set by the employer.
Neither council publishes a scale of its own. The HPCSA registers Clinical Associates and the SANC enrols Nursing Auxiliaries, but registration defines what you may do, not what you are paid.
The qualification, not the years, sets the band. Clinical Associate is a degree-level role with a wider scope than Nursing Auxiliary, so the Bachelor of Clinical Medical Practice is the step that changes your earnings.
No official scale is published for this role, so treat any single figure you find online as a third-party estimate rather than a rate you can rely on.
