How to become a registered massage therapist in South Africa
To practise in South Africa you register with the AHPCSA under the statutory title Therapeutic Massage Therapist. Both details are worth getting right at the outset: the regulator is the AHPCSA, not the HPCSA — massage therapy is one of eleven allied health professions under the Allied Health Professions Act 63 of 1982, and the HPCSA does not register massage therapists at all. Applications sent to the wrong council are a common and entirely avoidable delay.
What you need
A 240-credit qualification in Therapeutic Massage Therapy from an AHPCSA-accredited provider, plus a pass in the Council-approved bioethics and jurisprudence assessment. Applications are assessed under Regulation No. 127 of 12 February 2001.
The route
Complete the 240-credit qualification at an accredited provider. Apply to the AHPCSA for registration and sit the bioethics and jurisprudence assessment. Once on the register you may practise, and you must then maintain CPD under the Council's current guidelines.
What catches people out
The AHPCSA classifies Therapeutic Massage Therapy as a therapeutic rather than a diagnostic profession — unlike chiropractic, homeopathy or Chinese medicine, which are diagnostic. That distinction sets the boundaries of what you may claim and do, and it is not obvious from the qualification itself. Worth watching too: the AHPCSA has signalled an intention to open a register for Somatologists, which would bring part of the beauty-therapy massage sector under statutory regulation for the first time.

Therapeutic massage in South Africa is private practice, so pay is set by the practice that employs you or, once you run your own, by what you charge.
There is no public-service occupational class for the role, and the AHPCSA registers therapists without publishing any pay scale.
