How to register as a herbal practitioner in South Africa
South Africa has two entirely separate statutory routes here, with different regulators, different training and different scopes. Phytotherapy — Western herbal medicine — is a registered allied health profession with the AHPCSA. Herbalist is a registration category in its own right, but under the Traditional Health Practitioners Act 22 of 2007, administered by the Interim Traditional Health Practitioners Council. Which one you want determines everything that follows, and they are not interchangeable.
The phytotherapy route
Register in the Phytotherapy category with the AHPCSA. The qualification is a five-year accredited programme — three years of basic medical sciences plus a two-year specialisation — along with a Level One First Aid certificate and the Council's bioethics and jurisprudence assessment.
The traditional route
Registration as a Herbalist under section 21 of the THP Act, which recognises four practitioner categories: diviner, herbalist, traditional birth attendant and traditional surgeon.
What catches people out
Phytotherapy graduates who do not register with the AHPCSA within six months of graduating must additionally sit a competency assessment before they can register at all — a delay penalty unrelated to the quality of the degree. On the traditional side, the Act defines "herbalist" circularly, as a person registered as one under the Act, and the draft registration regulations were only gazetted in June 2024, seventeen years after the statute.
Herbal practitioner salary in South Africa
Herbal practitioners in South Africa work mainly in private practice under either regime, so pay is what the practice charges.
No official figure is published for either route; neither the AHPCSA nor the Interim Traditional Health Practitioners Council publishes a scale.
